Before you continue, please answer the questions below. If the answer is Yes to any of the questions, a Plan Check is required for this permit.

1. Is there any new installation where one or more services, switchboards, motor control centers, or feeders have a rating of 400 amperes or larger?

2. Is there any new installation of equipment or devices rated above 600 volts or any transformer?

3. Is there an assembly room or similar place having and assemblage or seating capacity exceeding 100 persons?

4, Is this a hospital or other health care facility with surgical operating rooms falling within the scope of Article 517 the National Electrical Code?

5. Is there any installation in locations classed as hazardous locations by the provisions of Chapter 5 of the National Electrical Code?

6. Is there any new installation of lighting fixtures or sound equipment weighing more than 250 pounds?

7. Is there any installation of fuel generated power equipment (e.g. generators). Note: AQMD Permit 400-A may be required |
